As part of the Maplewood Campus, The Homestead at Maplewood also provides priority access to Maplewood Care Center for long-term care and short-term rehabilitation.

  • Horizons Health Care and Retirement Community

    Horizons Health Care Center has been serving the community since 1972 and provides quality long-term care and inpatient/outpatient rehabilitation. Horizons Health Care is an integral part of the Surface Creek community of Cory, Austin, Eckert and Cedaredge. We are located in the shadows of the Grand Mesa on nine acres in rural western slopes of Colorado. We care for individuals with chronic diseases, and for those recovering from injury or illness.

    We have an 11-bed rehab wing with private and semi-private rooms dedicated to short-term rehab guests with our goal to enable them to return to their homes as quickly as possible. We also have a 16-bed memory support neighborhood for the care of individuals with Alzheimer's and other dementia-type illness who wander and/or need quiet structured environment. Our staff provides kind, compassionate care to our 'family', and we welcome residents' families and friends to be involved with their loved ones' care and activities.

  • Lambert House Adult Day Center

    Adult Day Centers are structured programs that meet the needs of functionally and/or cognitively impaired adults. They feature individually planned social and support services, supervision, and health monitoring in a protective setting during the day, and facilitate increased well-being of the entire family. Through the creation of a balanced system, caregivers are able to maintain home-based care.

    Our center provides:

    • Adult Day Services for seniors and disabled adults with therapeutic activities
    • On-site health care and caregiver support
    • Services for those with Alzheimer's or Parkinson's disease, dementia, or post-stroke
    • Disabled and special needs adults
    • We accept Medicaid and Oregon Project Insurance. Private pay costs are far less than assisted living facilities and in-home care. Scholarships are available
    • Attend as often or little as needed. Hours of Operation: 7:30 a.m. to 5:30 p.m.

    • Occupational therapy to promote independence in daily skills for dressing, bathing, cooking and homemaking to achieve a safe transition to home.
    • Speech therapy to restore communication and safe eating.

    Laurel Manor therapists offer coordinated services in a positive, caring and therapeutic atmosphere, promoting individual goals and treatment.

  • Mary Marshall Assisted Living Residence

    Partnering with the Arlington County (Virginia) Department of Health and Human Services to merge our experiences with seniors and people with disabilities and collectively deliver a unique program model, Mary Marshall offers an innovative, community-based community for seniors with intellectual disabilities and mental illness.

    The 52-unit licensed assisted living residence provides homes for low-income people age 55 and older with cognitive, mental health and other special needs who require assistance with activities of daily living. As assisted living programs have been more successful in allowing people with intellectual disabilities to live healthier, more fulfilling lives, these people are also living longer. As they reach old age, they face both their existing disabilities and new issues, such as limited mobility and vision, that come with aging.

    At Mary Marshall, all of our services are about helping those we support create whole lives. We are strongly committed to the principles of person-centered planning and individual choice.

    We help our residents focus on identifying their own strengths. We help them build upon their own personal assets, interests and desires. And our staff is able to support the health and safety needs of each person, including personal independence and assistance with activities of daily living, as needed. As part of a faith-based organization, Volunteers of America staff are fueled by love and compassion, and an unfailing dedication to the human spirit.

  • Nashoba Park Assisted Living

    Nashoba Park Assisted Living, located on the hilltop just above downtown Ayer, is a historic renovation of the former Ayer Community Hospital offering seniors gracious accommodations, a full range of supportive services and many amenities at reasonable monthly rates. Nashoba Park is a Volunteers of America Massachusetts Senior Community, managed by Senior Living Residences (SLR) featuring evidence-based arts, fitness and lifelong learning programs designed to help seniors stay sharp, social and independent.
