Rochester Rehabilitation and Living Center's memory support program offers a tranquil household where people with memory loss experience a structured schedule of daily care and activities, along with the everyday comforts of home. Specially trained staff are dedicated to the unique needs of each person, and attentively follow individualized plans of care. In addition, caregivers support family of those with memory loss through resources which include social services, Validation, the QuILT program and Alzheimer's support groups. Discreet managed-access technology throughout the memory support household allows for dignified personal monitoring.
From the moment our concierge greets you upon check-in, you'll realize the priority of our Guest Services team is helping you make the most of your stay. Guest Services team members are pleased when they can answer your questions or arrange for your requests to be fulfilled. Senior CommUnity Care of Michigan is a comprehensive health care choice for older adults living in Ingham, Eaton and Clinton counties and bordering areas of adjacent counties. Senior CommUnity Care of Michigan is a Program of All-Inclusive Care for the Elderly, also called PACE.
Senior CommUnity Care of Michigan is for:
- Older adults who want to live at home but need support.
- Caregivers caring for older adults living at home.
- Health professionals who want to help older adults living at home remain at home.
As a PACE provider, Senior CommUnity Care of Michigan helps people live independently at home with a high quality of life. Senior CommUnity Care of Michigan provides health care in the home and community to older adults who would otherwise live in nursing homes.
Senior CommUnity Care of North Carolina is a comprehensive health care program for seniors living in Wake, Durham and select zip codes in Granville counties and is a Program of All-Inclusive Care for the Elderly, also called PACE.

Last year, Senior CommUnity Meals served 117,000 meals to more than 2,600 older adults, covering 4,600 square miles in Colorado. Many of the older adults Senior CommUnity Meals feeds are the very farmers who fed our communities years ago, and the meals they receive through this program are the only food they eat. Hunger and good nutrition in older adults is a growing problem. Were it not for Senior CommUnity Meals, many older adults in this area would not have food to eat.

Meals on Wheels
Senior CommUnity Meals delivers hot meals to homebound program participants whenever possible. In some circumstances, such as for people in more remote locations, frozen meals are delivered. You must be 60 years of age or older and assessed as homebound to receive Meals on Wheels. Other criteria for eligibility apply in specific limited circumstances. We know that sometimes older seniors with health conditions or physical limitations can get lonely, and long for social interaction. In our Adult Day Services program, we offer social, recreational and supportive services to seniors who live alone, or as a dependent relative. Our clients enjoy nutritionally balanced meals and snacks, and a variety of activities including discussion groups, community outings, guest speakers, crafts, exercises, games, movies and parties.

Sleepy Eye Area Home Health provides a variety of medical services under the guidance of a registered nurse and your personal physician. Our goal is to maintain your independence and health in your own home. Sleepy Eye Area Home Health offers Home Care Services at Countryside Retirement Community, Ross Park Apartments and in the community of Sleepy Eye.
Services include visits to help with bathing, housekeeping and medication management. Frequency of visits is based on your needs and wishes.
Sleepy Eye Area Home Health also provides 24-hour assisted living services at Countryside Retirement Community and Ross Park Apartments.
We provide personalized health and home management services in the comfort of your home. A registered nurse works with you to develop a plan of care, and supervises staff to assure your needs are being met.
A registered nurse is available for a free visit to discuss your care needs, and services that are available to help you stay safe and healthy in your home.
Sleepy Eye Care Center is part of Sleepy Eye Campus, a Volunteers of America senior living and care community offering a full continuum of care. From independent living options to in-home and community-based health services, Sleepy Eye Campus includes Countryside Retirement Community, Sleepy Eye Area Home Health and Sleepy Eye Adult Day Services.

Sojourn at Seneca is a comprehensive inpatient behavioral health hospital designed for older adults. The main goal of treatment is to overcome current crisis, achieving personal growth in the process.
In collaboration with each patient's physician, our team of health care professionals designs an individual treatment program to address emotional and psychological health, as well as any outstanding physical and medical issues a patient is experiencing. Sojourn at Seneca's behavioral health program is specially designed to meet older adults' therapeutic needs, while offering a comfortable and welcoming environment during treatment.

Skilled Nursing Visits
Some of the typical client needs for skilled nursing visits include:
- Wound care
- Medication management
- Diabetic teaching and management
- Recovery from an illness or injury
Physical therapists evaluate daily activities—transferring from one location to another (e.g., from bed to wheelchair), walking and stair training—with a focus on safety, mobility, balance and endurance. They also provide progressive strengthening, balance and fall prevention strategies.
Occupational therapists assess self-care skills such as dressing, bathing and eating, as well as more complex tasks such as meal preparation, light housekeeping and adaptive self-care techniques.
Speech therapists assess and treat speech, language, cognitive-communication, voice, swallowing, fluency and other related disorders, and treat speech, language and swallowing disorders.
Home health aides work under the supervision of a registered nurse (RN) or therapist. Home health aides have received specialized training or certification as a home health aide, and meet state of Minnesota standards. Some examples of what a home health aide can assist with are:
- Personal care: bathing, grooming, oral hygiene, shaving, hair/skin/nail care and dressing
- Stand-by assistance with walking, transfers or a home exercise program set up by a therapist.
